Qui tam cases filed under the Federal False Claims Act are filed in federal district courts and investigated by United States Attorney’s Offices in the districts.

Illinois

Illinois has three federal district courts, U.S. District Courts of the Central, Northern, and Southern District of Illinois.

The Central District of Illinois has Peoria, Urbana, Springfield, and Rock Island locations.

The Southern District of Illinois has court locations in Benton and East St. Louis.

The United States District Court for the Northern District of Illinois has court locations in Chicago and Rockford. The U.S. Attorney’s Office for the Northern District of Illinois announced a settlement that resolved a qui tam case that alleged an unbundling scheme where doctors performed multiple mole removal procedures on patients on a single date but submitted claims as though the procedures were on multiple dates.

Indiana

Indiana has two district courts, the Northern and Southern districts of Indiana. The United States District Court for the Northern District of Indiana has court locations in Fort Wayne, Hammond, Lafayette, and South Bend.

The Southern District of Indiana has court locations in Indianapolis, Terre Haute, Evansville, and New Albany. In December 2023, the Southern District of Indiana announced a $345 million settlement to resolve allegations of Stark Law violations where physicians were recruited for referrals and compensated at above-fair market value rates.

Iowa

Iowa has two district courts, the Northern and Southern districts of Iowa. The Southern District of Iowa has court locations in Des Moines, Davenport, and Council Bluffs.

The United States District Court for the Northern District of Iowa has court locations in Cedar Rapids and Sioux City. The United States Attorney’s Office for the Northern District of Iowa announced a settlement with a manufacturer for failing to conduct contractually required testing and falsifying test results for parts used in military aircraft.

Kansas

The District of Kansas is the only federal district court in Kansas. It has court locations in Kansas City, Topeka, and Wichita. On December 13, 2022, the United States Attorney’s Office for the District of Kansas announced a $3 million False Claims Act settlement involving a long-term care pharmacy dispensing controlled substances without valid prescriptions.

Michigan

The Western District of Michigan has court locations in Grand Rapids, Kalamazoo, Lansing, and Marquette.

The Eastern District of Michigan has court locations in Detroit, Ann Arbor, Bay City, Flint, and Port Huron.  In October 2023, the U.S. Attorney’s Office for the Eastern District of Michigan announced a $4.4 million settlement to resolve allegations that the defendants upcoded inpatient hospital services and billed for services not rendered.

Minnesota

The U.S. District Court for the District of Minnesota has locations in Minneapolis, St. Paul, Duluth, and Fergus Falls. The U.S. Attorney’s Office for the District of Minnesota announced a $4 million settlement for allegations that a physical therapy company submitted false claims for outpatient physical therapy services not provided.

Missouri

Missouri has two district courts, the Eastern and Western districts of Missouri. The Western District of Missouri has locations in Kansas City, Jefferson City, and Springfield.

The Eastern District of Missouri has court locations in St. Louis, Cape Girardeau, and Hannibal. The U.S. Attorney’s Office for the Eastern District of Missouri announced a $1.86 million False Claims Act settlement where a behavioral health provider made false claims for services not provided to foster youth.

Nebraska

The United States District Court for the District of Nebraska has court locations in Omaha, Lincoln, and North Platte.

North Dakota

The United States District Court for the District of North Dakota has court locations in Bismarck, Fargo, Minot, and Grand Forks.

Ohio

Ohio has Northern and Southern district courts. The U.S. District Court for the Northern District of Ohio has locations in Akron, Cleveland, Toledo, and Youngstown.

The U.S. District Court for the Southern District of Ohio has court locations in Cincinnati, Columbus, and Dayton. The U.S. Attorney’s Office for the Southern District of Ohio announced a settlement that resolved allegations that a home healthcare provider defrauded the government by billing for individual healthcare services when the services rendered were provided in a group setting.

South Dakota

The U.S. District Court for the District of South Dakota has court locations in Sioux Falls, Rapid City, Pierre, and Aberdeen. The Department of Justice announced a $9.2 million settlement resolving allegations that a medical device manufacturer induced a South Dakota neurosurgeon to use its products by paying kickbacks.

Wisconsin

The United States District Court for the Western District of Wisconsin is in Madison, and the Eastern District of Wisconsin has locations in Milwaukee and Green Bay.

The U.S. Attorney’s Office for the Eastern District of Wisconsin announced a $3.5 million settlement that resolved allegations that a pharmacy violated the False Claims Act by submitting claims to Medicaid for stimulant medications without complying with Medicaid rules.
